TERMS & CONDITIONS
Membership in the 180 Healthcare, LLC (“180”) is conditioned on acceptance by you of the terms and conditions contained herein. By purchasing a membership to 180 or using or continuing to use the benefits of 180, the Member is deemed to have agreed to all such terms and conditions of the Agreement.
SECTION 1: ELIGIBILTY AND ENROLLMENT
1.1. You must be 18 years old or older to enroll in the Membership for yourself and your family.
1.2. You must provide and maintain accurate and true personal information, which includes: a demographic sheet, medical history, consent form, photo ID and billing information, when applying for the 180 Membership or using any 180 benefits. Failure to do so may invalidate membership in 180. It is your responsibility to update personal information by contacting the home office with any demographic, financial changes or medical history modifications. The home office is located at 24 North Sprigg, Suite B, Cape Girardeau, Missouri and the telephone number is (573) 803-4661.
1.3. Membership in 180 is non-transferable. Only you or your family may use the benefits of Membership in 180. You must promptly notify 180 upon becoming aware of any unauthorized use of the Membership.
SECTION 2: MEMBERSHIP
2.1. Your Membership will include the following benefits:
a. Access to the health care providers.
b. Telehealth services provided by 1.800MD.
c. Discounted generic and name brand prescription medications
d. Lab work at discounted rates.
e. Snap Fitness membership to adult Members.
2.2. Health Care Providers. 180’s health care providers will provide certain health care services, and the providers and services are subject to change without notice. 180’s health care providers are available to Members by appointment only though its home office. Members will be seen by a family nurse practitioner and/or a physician. Appointments are not limited, however, if it is determined by 180 that a Member is abusing the privilege by making unnecessary appointments, 180 reserves the right the right to discontinue your membership. All office visits must be scheduled through 180 Healthcare’s central office located at 24 N Sprigg Street Cape Girardeau, Missouri 63701 Phone: 573-803-4661. Cape Family Medical Clinic is the PRIMARY health facility for 180 Healthcare LLC. When a member calls for an office visit they will be scheduled to see a provider at this location first since this location is where members will be seen for the unlimited office visits. MedStop One and Jackson Medical Center are limited visit facilities who will see 180 members on an appointment basis only. There will be a limited number of appointment slots opened every day for six days Monday through Saturday according to their hours of operation.
2.3. 1.800MD. Telehealth services provides every Member access to non-emergent care. Upon enrolling in 180, Members will receive a welcome e-mail from 1.800 MD. Members are responsible for registering with 1.800MD. After the Member completes his or her registration, 180 will send your demographic information to 1.800MD. 180 will assist, if requested, with this registration during regular business hours of Monday – Thursday 8:00- 5:00. You may also contact 1-800-530-8666 to receive help from 1.800MD for this part of the registration.
2.4. Generic prescription medications. A local pharmacy will be providing generic prescriptions at $4.00 per prescription. For more information, visit or call 180 Healthcare. Generic prescriptions and pricing are subject to change at any time without notice. This is a service that is offered by a local pharmacy to 180 Healthcare members and is subject to change without our notice.
2.5. Lab Work. 180 have negotiated certain lab services at discounted rates with the various providers that are contracted with 180. Lab work and pricing are subject to change at any time without notice. If a member has a question about a certain lab test or lab price, it is the member’s responsibility to ask the provider for more information before such test is performed.
2.6. Snap Fitness 24-7. 180 Members will have access to a Snap Fitness 24-7 gym membership located at 2530 William, Cape Girardeau, Missouri. 180 will be happy to work with Members in order to get them a tour of the facility and to answer any questions they may have about services provided. Your 180 Membership does not include the initial card fee of $20.00. Any lost or stolen cards will cost $15.00 to replace. Card fees are subject to change by 180 or Snap Fitness. Children 13 or younger are NOT permitted in the gym. Youth Members who are 14-17 years of age must have a separate gym membership for youths/minors, must be accompanied by a family adult, and must have his or her separate Snap ID card. This gym membership is governed by Snap Fitness 24-7 Terms and Conditions.
2.7 Services provided under membership agreement consist of but are not limited to the following items listed on the benefits page located on the website. While there are many services included in the membership there may be additional costs associated with injections of certain medicines. Members will have the choice to accept or deny any injectable medicines that the providers may suggest or deem necessary to treat members condition. Issues that are of not medical necessity or considered cosmetic treatments/procedures such as weight loss, Botox injection and laser hair removal …etc. are not covered under the monthly membership fee.
SECTION 3: PAYMENT
3.1. Upon enrollment, you will be charged the Membership Price, the value of which is $99.00 per month per person age 18 and over, and $10.00 per month per person under the age of 18, which will be due on the anniversary date of each month.
3.2. Charges of the Membership Price are non-refundable.
3.3. The Debit Card, Credit Card or Auto Bank Withdrawal you use for your Initial Membership Price will be retained as the default payment method.
3.4 Member will be responsible for payment of all services performed that are not covered under the 180 Healthcare membership agreement. These services include but are not limited to Labs, Injectable medicines, and other ancillary services. Member at the time of office visit will have the choice to accept or refuse any such services that may be offered to them as treatment for their condition. Since these services are performed in the office during the member visit the member will be expected to pay cash before such services are rendered.
SECTION 4: MEMBERSHIP TERM AND RENEWAL
4.1. 180 reserves the right to cancel, extend, or suspend Membership at its sole discretion for any reason.
4.2. The Membership term is six months and will automatically renew for successive terms. Members will not be able to cancel their Membership during the current term. Members may withdraw from their Membership by providing written notice to the home office within 30 days of the end of the current term.
4.3. Recurring Membership is effective for a period of six months following the renewal date in each succeeding period.
4.4. The Renewal Price will be assessed to the default payment method you authorized.
4.5. If 180 is not able to complete a charge of the Renewal Price, you will lose Membership benefits and you must enroll again.
4.6. The Renewal Price is subject to change by 180 at any time without notice. Any increase in the Renewal Fee will take effect upon the following renewal term.
4.7 Members who cancel or terminate early their Membership will be assessed a re-enrollment fee of $159.00 upon renewing.
SECTION 5: LIMITATION OF LIABILITY
5.1. 180 accepts no liability for any losses or claims arising from any Member unable to access the Membership Benefits under 2.1, nor the actual treatment or services provided by a third-party providing Membership Benefits under 2.1. 180 shall not be liable to a Member by reason of any delay in performing, or any failure to perform, any Membership Benefit.
5.2. Member agrees and understands that he or she will be responsible for their acts, errors, statements or omissions (including family and/or any non-member accompanying the Member) at any place where a Membership Benefit may be provided, and hereby indemnifies 180 against any and all demands, claims, actions, liabilities, damages, losses and/or expenses from same.
5.3. This section applies only to the extent permitted by law.
SECTION 6: WARRANTY
6.1. 180 makes no warranties, express or implied, guarantees or conditions with respect to your use of the Membership Benefits. You understand that the use of the Membership Benefits is at your own risk. 180 doesn’t guarantee the Membership Benefits provided through its third-party providers, nor the accuracy or the timeliness of information by a third-party.
6.2. This section applies only to the extent permitted by law.
SECTION 7: CHOICE OF LAW AND JURISDICTION
The Member and 180 irrevocably consent that laws of the State of Missouri shall be used and to the exclusive jurisdiction and venue of the state or federal courts in Cape Girardeau County, Missouri, for all disputes arising out of or relating to these Terms and Conditions.
SECTION 8: MISCELLANEOUS
8.1. 180 may modify these Terms and Conditions at any time without notice.
8.2. These Terms and Conditions, and any other referenced source herein, shall constitute the entire agreement between the Member and 180.
